STUDENT REGISTRATION – Southern Oaks Middle School<\/strong><\/p>\n

Once the student has been assigned by the St. Lucie Public Schools Student Assignment Office to Southern Oaks Middle School, the forms below are to be completed and brought to the school or emailed to our Registrar, Mara.marrero@stlucieschools.org<\/a><\/p>\n

Right click on the link to download forms. These forms can be filled in on the computer, saved, and then returned via email. If you prefer to hand-write, click on the form and print. The school will need a copy of the following documents with the completed forms.<\/p>\n

Any parent or guardian registering students in St. Lucie Public Schools for the first time must upload the following documents when registering for school.<\/p>\n

Documents Needed:<\/strong><\/p>\n

\u00b7 Original birth certificate.<\/p>\n

\u00b7 Health Examination (physical) The exam must be dated within 12 months prior to enrollment. Not required for students transferring from any Florida public or private school, if the requirement has already been met.<\/p>\n

\u00b7 Florida Certificate of Immunization (original blue card). If you have moved from another state, you will need to take your current\u00a0 \u00a0 \u00a0 immunization record to the St. Lucie County Health Department (5150 NW Milner Dr., Port St Lucie, FL 772-462-3800), to your doctor, or to a clinic to have immunizations transferred to FL-680 Form.<\/p>\n

\u00b7 Social Security Card (if available).<\/p>\n

\u00b7 Proof of custody (if the child is not living with both natural parents).<\/p>\n

\u00b7 Copy of Individual Education Plan (IEP) if your child is in an Exceptional Education Program.<\/p>\n

\u00b7 Picture ID of parent\/guardian (MANDATORY).<\/p>\n

\u00b7 2 Primary Proofs of Address (NAME and ADDRESS must be printed on document):<\/p>\n

\u00b7 Electric, Water, or Land Line Telephone Bill \u2013 within 30 days.<\/p>\n

\u00b7 Signed Lease Agreement \u2013 within 60 days.<\/p>\n

\u00b7 Official Rent Receipt \u2013 within 30 days.<\/p>\n

\u00b7 Current Mortgage Deed \u2013 within 60 days.<\/p>\n

\u00b7 Mortgage Payment Coupon \u2013 within 30 days \u2666 Sales\/Builder\u2019s Contract \u2013 (with Completion within 6 months).<\/p>\n
