Every athlete needs to complete an Athletic Packet prior to participating in any of our sports.

The Athletic Packets are available in the Athletic Office or parents can print the packet here:

• FPWA Athletic Packet
• Form GA4  (If transferring from another High school)

• Athletic Processing Fee – $50 FOR 1 SPORT, $100 FOR 2+ SPORTS
